Parent / Guardian Information

Because the applicant is under 18, this is a Junior Membership application. The identification documents below must belong to the parent or legal guardian.

Parents and guardians are welcome and encouraged to join REACT alongside their child. A parent/guardian may submit a separate Adult Membership application using this same form.

Emergency Contact

Provide someone we should contact in an emergency. This is an emergency contact, not a next-of-kin designation. For Junior Membership, the parent/guardian details are filled here automatically by default and may be changed if a different emergency contact should be used.

Applicant Identification

Add at least one identification document belonging to the applicant.

If an ID has a front and back, add the same ID twice using Add Another ID. Select the same ID type and enter the same ID number, issue date and expiry date on both entries, then upload the front image on one and the back image on the other. The two sides will be combined when the membership record is created.
